NCLEX Psychiatric Medications: The High-Yield Drugs and Side Effects to Know (2026)

Psych medications combine two of the most tested NCLEX areas, mental health and pharmacology, which is why they appear so often. The exam focuses on a manageable set of drugs and a few critical safety findings. Master these and you cover most psych pharmacology questions.

Antidepressants

  • SSRIs: first-line, take weeks to work (a key teaching point), and watch for serotonin syndrome, especially when combined with other serotonergic drugs.
  • MAOIs: the tyramine diet is the classic NCLEX point. Avoid aged cheeses, cured meats, and certain fermented foods to prevent hypertensive crisis.
  • Serotonin syndrome: high fever, agitation, tremor, and autonomic instability. Recognize it fast.

Mood stabilizers

  • Lithium: the most heavily tested. Narrow therapeutic range, so know the levels and the toxicity signs (tremor, GI upset, confusion, and at higher levels seizures). Adequate sodium and fluid intake matter because changes affect lithium levels.

Memory point: if a lithium client becomes dehydrated or sodium-depleted, lithium rises toward toxicity. The NCLEX tests this relationship constantly.

Antipsychotics

  • Extrapyramidal symptoms: know the spectrum, from acute dystonia to tardive dyskinesia.
  • Neuroleptic malignant syndrome: a medical emergency. High fever, rigidity, altered mental status, and autonomic instability.
  • Clozapine: the agranulocytosis risk means regular blood monitoring, a favorite test point.

Anti-anxiety

  • Benzodiazepines: effective short-term, but watch for dependence and respiratory depression, and know flumazenil as the reversal agent.

The pattern the NCLEX wants

For psych meds, the exam almost always tests one of three things: the dangerous syndrome to recognize, the teaching point (onset time, diet, monitoring), or the lab or level to watch. Frame every psych med around those three and you will answer with confidence.
